Payment Requirements:  Carnival requires a deposit of $75 per person to secure cabin
accommodations.  Deposit is due with submission of this registration form.
August 2, 2008:  $100 payment due to Carnival Cruise Lines.
October 2, 2008 Remaining balance is due to Carnival Cruise Lines/Mary Roth.

I hereby authorize Carnival Cruise Lines to charge my credit/debit card $75. for my
deposit for the Mary Roth CE @ Sea Cruise aboard Carnival Ecstasy departing

Cancellation and Refund Policies:  If you cancel, Carnival will asses the following charges.
(Because travel insurance is included in the price of this cruise, you may be reimbursed
if you cancel for a "covered" reason).

     •  Until October 2nd --- Full Refund
    
•  Oct. 3rd --- Nov. 3rd --- $175 is non-refundable
    
•  Nov. 4th --- Nov. 25th --- 50% of total fare is charged
    
•  Nov. 26th --- Dec. 4th --- No Refund

IMPORTANT:  Required Travel Documents: 

Carnival STRONGLY recommends that all passengers have a passport.  Other documents
(i.e. Original Certified Birth Certificate and Drivers License) may not be accepted at check-in.

Extremely Important:  If you arrive  without proper documentation, you will not be allowed to board the ship!
No refund will be granted to anyone arriving on the day of departure without the the proper documents.
